Fall Registration Info

2009-2010 Rialto Youth Soccer League Fall Registration

(Be sure to scroll all they way to the bottom of the page.)

Registration time for ALL dates (except as noted) is 9:00 am - 2:00 pm

  • May 9th
  • May 16th
  • May 30th
  • June 6th
  • JUNE 13th from 9am -12pm has been added 

Registration will be held at Jerry Eaves Park.

Registration fee for all ages is $100 per child. $90 for the 3rd and up child in immediate family.

$30 worth of raffle tickets will be given to each player which they can either sell or keep.
If you sell the tickets, you keep the money.
All ticket stubs must be turned in to your child's coach prior to opening ceremonies.

$2.00 transaction fee for debit/atm and credit

Original or original certified copies of birth certificate required at time of registration

To request a certified copy of a birth certificate, scroll to the bottom of the page at this link to download and print the form. Photo copies of birth certificates cannot be accepted.

There will not be any special requests granted this year, which include carpool, extended family on same team, etc.

This form can be completed online by moving your cursor to the top of the form where the years of the season are located (20__ to 20__). Use your cursor to move from location to location on the form.

3 copies must be printed and brought to the park with you when you register.

If you would like to volunteer as a coach, assistant coach or a team parent, please complete this form, print 3 copies and bring them to registration with you.

You will not be able to save the completed form to your computer without the full Adobe Acrobat software.

Blank forms will also be available at the park.

These fees include each player's jersey, shorts, socks, pictures, insurance, player participation trophy and a small portion being deposited into the referee and snack bar fund.

The monies that are set aside for Snack Bar Fund are returned to the teams to be used for their team parties once the following condition is met:

  • Each Team Must Provide 6 - 8 People (age 16 or older) To Work A 4-Hour Block In The Snack Bar.

The monies set aside for the referee fund are used to pay our referees for their services.

The season consists of a 12 game schedule which may be extended at no additional charge if your child's team goes on to the Commissioner's Cup or if they are selected for the All-Star Tournament.

The player's parent or legal guardian must be present at registration. You will need to bring your child's birth certificate or valid visa.
